This is how it can be done.
1. Start PowerPoint and insert the picture to a new slide

2. Add texts, arrows, labels, etc by using the Drawing toolbar in PowerPoint

3. Once completed, click Edit on menu bar and choose Select All

4. Right-click on the picture and select Save as Picture... from the context menu

5. Choose JPEG or other image file type and Save

Done! You’ve just created a picture with annotations!!!
